This man is 100 percent!!! He saved me with a bad tattoo that was a cover up, so then he had to go in and do a cover up of a cover up!!!! Came out so amazing and my money will be spent in this shop for now on!!! Bo!!! Your the gosh dang bomb! Thank you for great service!

Bo has done many of my tattoos now and he always turns out great work! He has done my girlfriends and sisters tattoos and they are all very happy with them. He is very reasonably priced and well worth every penny!

My family has been going to Ultimate art form for years. Me personally close to 20 years and now....Ive taken my kid. Bo and the staff are the best. The shop has the an awesome selection of Body Jewelry and you can purchase extra piercing and tattoo healing products if you run out too early. They are always top notch and by the book. Bo is awesome and amazing we love him to the moon and back for his great compasion and kindness in helping my son fix his wheelchair when it fell apart by his shop and all the donations to our friends funeral fundraiser! Also he is just plain hilarious and if he cant make you laugh then youre probably just not a person with a good sense of humor and shouldnt leave your to house because he can and will turn around a frown upside down with his contagious $#*+ eating grin! I am late to write this review, but I got my tattoo near the end of August and it finally healed about a week ago. I can say first hand my experience was amazing. BO is suoer friendly and fun to talk to. He makes the experience of getting a tattoo much easier and doesnt overcharge. The shop is super clean and sanitized between each customer. Last time I spoke to BO he was running the place by himself, which is super impressive and awesome. Would recommend to anyone looking for a great artist.

Bo has done two of my tattoos now and they have both turned out AMAZING!! I will never go to anyone else. He is very experienced and can fill all your tattoo wants/needs! He responds quickly and sets you up with an appointment as quickly as he can! His shop is very peaceful and relaxing.

My experience here was great! I came in for a walk-in tattoo and and was helped immediately by the owner Bo. He was very professional and personable. I got a tattoo on my butt cheek and he even put up this little curtain so nobody could see me if they walked in. He made me feel very safe while i was there and the work he did was absolutely amazing. To make it even better the shop was actually a comfortable temperature! Ill definitely be back soon and highly recommend that everybody comes and pays Bo a visit as well :)

Bo was phenomenal. I got my belly button pierced and he was quick and comforting. i am extremely scared of needles but he made me feel super comfortable. everyone there was nice and welcoming and extremely funny. great environment, awesome establishment! i definitely recommend:)

My overall experience here was amazing! I had my septum pierced by Bo and he couldn’t have done any better of a job! I was extremely nervous and he made me feel comfortable and talked me through the whole process! After he had pierced me he walked me through all the steps to make sure I knew how to flip it up if I needed to hide it for work and how to properly keep everything clean. Everyone in the shop was super friendly as well! I will for sure be going back for any future piercings!

Bo was amazing. I went in a couple weeks ago to buy jewelry and even then the staff was knowledgeable and fun. Went back yesterday with a friend to get piercings and it was the same; laid back and fun but professional. Staff was talking to each other and laughing which made us laugh with them! Price was very reasonable, especially for multiple piercings on one person. We were both happy with our piercings. Highly recommend!

Bo both of my first two tattoos, along with two of my boyfriends, and pierced my belly button. The tattoos look amazing and I will never go to another shop! He is so experienced and it definitely shows in his work. Prices are very good for the work! Worth every penny! My best friend and I went in for matching Rook piercings and can not say enough about our experience. Tracy was so knowledgeable, kind, and patient with us. His gentle nature and his calm demeanor helped me through my anxiety of my first piercing. Very affordable. The business was clean. We were in and out in no time!

My son has been here in the past and his tattoo was good. So I thought when my son passed away suddenly in a motorcycle accident I would go here to get my memorial tattoo. Whomever the gentleman was that waited on me the day I went in was not the same man that did my son’s tattoo. Anyway I asked if he could do a memorial tattoo, he said October 11 was his first availability. Well, if you have ever lost a child, that is just too long to wait. I asked if he would recommend anyone in the area? He said ‘not with something that important’. Side note, Im not sure he had a heart in there. My thing is how are you a tattoo artist and don’t know any other talented tattoo artist?!? I am a good nurse but I also know a lot of other good nurses. Anyway, I went to The Cooper Fox in Wright City and Alexis did an amazing job! So…now you know another talented tattoo artist…you are welcome.

Called ahead about a possible walk in same day. Came in a few hours later and they were able to get my husband right in. Took about an hour start to finish. Guy did a great job! Reasonable prices, VERY clean and professional! We will be back!

I was recommended to see Bo by a friend and he did not disappoint. We only needed simple second ear piercings, but I knew I didn’t want to go to Claire’s! I wanted a professional! He went out of his way during the busy holiday season to fit my family in for ear piercings! Came in on his day off and prior to a family obligation. Was professional, funny, quick and made the entire experience painless! My niece was slightly scared to get hers done and he put her at ease! Highly recommend Bo and his work!

ZERO STARS -Such a joke. Disgusted annoyance tone the whole time talking to me... Then when I called to come back for him to look at it he was even more annoyed and said theres nothing I can do to make it look better. Hands down the WORST thing I have ever seen.. I have never gotten a tattoo before so I researched everywhere around and ended up finding this place due to the best reviews and I just gotta say they definitely lived up to the expectations! I’ve been in other tattoo shops with my brother and felt verbally harassed so my experience with tattoo shops were not well. However, this place made such a good impression on me that even when I move to FL I might have to schedule my next tattoos for when I visit my friends here in MO, that way I know I’m getting good work done with good people. Would highly recommend. 10/10, 5 stars!

I’ve gotten 4 tattoos with Bo from the Ultimate Art Form in the past 2 years and am so happy with how all of them have came out! He doesn’t pressure me into getting anything I don’t like/love unlike some tattoo artist do. I will forever go to Bo and no one else. I would wait serval months just to get tattooed by him.

Waited over 12 years to get my first Navy tattoo and when i brought my wife in to get a piercing i knew this was the perfect place to get it! Old school feel with real artists doing the work.Bo did my tattoo like it was on his own arm. Spent almost 6 hours on it with maybe one break. He was very meticulous and took his time. Got every detail right. Jimbo kept me laughing and treated me like family. Great environment and will be going back for every tattoo i get in the future! My sister and I got studs in our ears yesterday. This was my second ear piercing and I’m 17 so naturally I was a bit less nervous, but he measured very carefully and was very nice and they look great. Next up was my sister who is 12 and was getting her first. She was very nervous and he talked to her and made her feel comfortable and was very good about not pressuring her or anything and she ended up getting them and is in love with them. He was also very accommodating with me, my mom, and my youngest sister comforting her through the process as she was sitting for the piercing. Thank you so much for this experience I will definitely be back for all of my future piercings.

I have gotten both of my tattoos and one piercing from here! While getting my first tattoo and my nose piercing here I had gotten very nauseous and Bo was so nice and got me juice and a sucker. My most recent tattoo (one week ago) took Bo about 5 minutes to do! Everyone there is amazing and Bo is no doubt the best tattoo artist I have ever known.

I have been going to ultimate art form since the age of 17. Bo and his crew are simply amazing. I just recently went in to have my kids foot and hand print done with their names and it was knocked out of the park. The shading and detail is exactly spot on with the pictures of the day they were born. He is working one up for my son right now and I cant wait to see the art and talent that goes with it. This place has the best staff ever. I got my first body piercing here and... well I passed out from not breathing right; however, the staff took very good care of me and made sure I was absolutely fine before I left. We always go to Ultimate Art Form! Have been clients since we were 18 so 15 years now. Bo has done many piercings on my husband and myself as well as tattoos. We have also been tattooed by Jimbo! They are amazing artists and are invested in the quality of their work. For a truly customized and impressive tattoo I would recommend going nowhere else!

They are beyond amazing and so nice.I brought my nephew with me and they were 100% okay with it and even played with him. I highly recommend them. The only problem I had was I got an industrial bar, and it was $50 but it wasnt the metal bar, its plastic and its driving me insane lol. I thought for $50 Id get the metal one. But other than that I 100% love them. You can tell they really care about their customers.

Bo did magic on an awful tattoo of mine. The one part I really disliked on my old one turned out to be absolutely beautiful with his skill. I didnt give him any directions, just help make it look nice and I am beyond happy! The value for your money is so worth it with a lifetime art piece.

So first and foremost Im glad this is a public forum. I was actually in the store as a individual customer behind this customer and can attest to the so called truthful response of the owner. I too was in your store at the exact same time. So Im GOING TO CLARIFY @ the owner you stated the artist was on the phone...that is a OPEN LIE. The artist was not on the phone and was actually tattooing a patrons LEFT LEG the entire time. Its apparent that the owner in how he has handle both (1 star reviews) in the past month has no concept on respect, integrity or professionalism. So its going to be amusing the colorful and highly jaded response I receive.As for my experience I too was not ACKNOWLEDGED which was why I was going to file a review. Its not the quality of art the business delivers nor the selection. Its was simple lack of customer service that was not provided which by the way is a simple training issue as the owner. However @ the owner who has failed to grasp the concept stated has only shown he leads by example as the owner by being overly aggressive to negative feedback. AGAIN @ the owner complete failure of customer service. Point proven...so yes like that sign that hangs on the (right side wall in the middle before you reach the display of belly piercings)...since integrity isnt the owners strong suit. Its states cant compete with others prices for their work is their worth. Well @ the owner is your job as the owner your work? So your response to these things are your worth.. 1 stars are equally important as 5 stars ( Good luck removing your ignorance from the public eye) Fyi also I wonder how the lady that entered the shop briefly before I left experience went because she too was unacknowledged the entire time I was present. (Unless owner wants to says she never existed too...?)

I am currently working on a sleeve and have been going to this shop for 11 years. I will never go any place else. I am constantly getting compliments from strangers on how beautiful my tattoos are. Bo and Jimbo are knowledgeable and friendly and great at what they do. You will not be disappointed if you get ink or piercings done here. Ive had 2 piercings done here and my daughter has had 2 as well. This shop cares about its customers and thats hard to find these days. Check them out if youre looking into ink and/or piercings!

I heard this was the place to go for tattoos and I was very happy with my decision. I was nervous going in for my 3 hour appointment but Bo made me feel relaxed and I truly enjoyed my experience. Got my thigh tattoo done by Bo. I believe he did a really good job on it because I love it! Jimbo definitely kept me entertained while Bo worked on my leg which made the time go by faster. I have a couple more tattoos I am wanting to get soon and I will be going back to Ultimate Art Form to get them done.

I had my sons name tattooed on my foot by Bo and I absolutely love it. He also pointed out and later proved with his work that my previous tattoo for my daughter wasnt done well at all. I am planning to have it redone by Bo once the other foot heals up. Great experience overall.

I have been wanting a cover up done on my shoulder for years. I went to several tattoo artist in the area and none of them would touch it. Bo was so great and spent lots of time showing me ideas that would work and together we came up with the perfect cover up. The shop is super clean and I he was always super professional. I have now recieved 2 tattoos from Ultimate Art Form and both times they were amazing. Both of my tattoos are exactly what I wanted. As far as customer service, they are professional and have standards. All of the tattooing equiptment was new and sanitation is a big deal for them unlike some of the other tattoo shops around here, which as a customer there was a big deal for me. While I was there other customers were coming in and asking them to draw up and sketch original tattoo ideas and I was looking at their previous work for others and it was beautiful, detailed, and you could tell that they really value their clients because the detail in the works really put a persons story and personality into the art. I plan of getting more tattoos and piercings and I will not go anywhere else to get mine. Thank you Ultimate Art Form!

I reached out to UAF on Facebook about getting my 8-year-old daughter’s ears pierced. I was provided some decent information but nothing about needing to bring in a birth certificate for this process. After sitting and waiting for 30 minutes, I was informed that we would need my daughter’s birth certificate. Of course not knowing that information and not having her birth certificate, we were forced to leave. Maybe I’m just not well-versed on the requirements in today’s world, but I would encourage the business to share all necessary information from the onset as to not create a less than pleasant experience for your customers.

Daith Piercing for Migraines - Tales From a Non-Rebel, 40-Something Mother of 2.I have no tattoos. Heck, Id never even been to a tattoo parlor! I had my ears pierced at age 13 at the mall but that was a looonngg time ago. Ive gotten migraines since I was a teenager and while there are various triggers, one of my biggies is from sinus and air pressure changes. I heard about daith piercings by chance from a checker at the grocery store. She had horrible migraines and said she hadnt had one in 2 years after getting the piercing! Im not one to do something crazy like a piercing without checking it out first so besides researching it online, I asked my primary care Dr. He said there wasnt a lot of scientific data about it but that he had a few patients who had tried it with success. The risks were the same as getting your ears pierced - they can get infected if you dont take care of them. Still uncertain, I started a search for someone to do the piercing. I checked websites, reviews on multiple sites (Google, Yelp, Facebook) in the entire STL metro area and found that the best choice was in my own backyard of Wentzville - Ultimate Form Tattoo Studio. Since I am thorough in my research (plus a wuss!), I called to ask my list of questions first. I knew that Bo did the piercings but it was Jimbo who answered the phone. (Jimbo who is the definition of character. He could sub for a member of ZZ Top if needed.) He said that they had done about 200 of these piercings for migraine sufferers and that about 97% had complete relief. Those are some good odds! I could tell that Jimbo was not the kind of person who would blow sunshine up my skirt. Hes a straight shooter. I decided to get both ears done since I have pain on both sides of my head. It was $30 to do 1 ear or $50 to do both. They got me in about an hour later. I wanted to go before I changed my mind.The studio had the flavor that I had expected - pin up models with tattoos (they were tasteful though!), Harley motorcycle stuff and the pencil holder of a butt - complete with the pencil sticking out. The place was very, very clean though. I could tell that they took things like germs really seriously! Bo is a little more quiet than Jimbo but has a great sense of humor. I could tell right away that he knew his craft very well and I didnt have to worry about anything. I didnt have a migraine when I went but my head felt like a balloon because of sinus pressure. He pierced the first ear. It didnt hurt any more than getting a shot. Then the most bizarre thing happened - that side of my head completely cleared within a couple of seconds! By comparison, the other side still felt like a balloon! I didnt expect that at all! Same thing happened on the other side after he pierced it! I dont know if this is normal thing or not. Maybe I am an airhead like my husband has dared say before!Its been about a week since my trip to the wild side. Not only have I not had a migraine, I havent had any sinus pressure either. Its May in STL and pollen counts are off the charts - but my sinuses are clear as can be! Its not all fun and games though. My ears are sore like if you have earbuds that are too big in your ears too long. I have to clean them several times a day and move the rings so they dont get stuck. I get a little dizzy when that happens! I also get a headache after moving them but they arent bad and go away quickly. I know that will get better as they heal. So would I go and get more piercings for aesthetics? Nope! Would I do it all over again for the migraines? In a heartbeat! I was lucky to have found a place that gave me such a good experience though. My 19 year old percussionist son wants to get a tattoo. Hell be going to see Bo or Jimbo when hes ready!UPDATE: Its been 2 months since my piercings and still no migraines!! *WOO-HOO!!!

I am writing this review for my daughter. Bo did a helix piercing on her today. She was very nervous about the pain, it appeared effortless for him! She didnt flinch at all!

Shania did a fantastic job piercing my 11 year old daughter’s ear lobes. Probably a basic thing for Shania, but was a big deal to my daughter. Shania was kind and professional and my daughter felt no pain at all. That was about 10 days ago and all is still well, no post piercing issues at all! Thank you Shania!

Nice friendly staff. They changed locations and it has made an improvement. Though once I went to the old location and asked for some lettering and the sketch they put on my shoulder had a misspelling so I didnt get that tattoo. As long as BO is there. Youre gonna get a tattoo from a legend.

I was recently inked twice on my chest - three weeks apart - and each tat healed beautifully in just a couple of days. No swelling, scabbing, etc. Brilliant, clean work. Good artists. Check them out.

I highly recommend! I was able to walk in get my daith pierced within a recommended amount of time. Gentleman was super nice and place was very clean.

At 21 years in business are the oldest shop West of the Mississippi River. Bo Hayes the owner and has been here 20 years of that time. Jimbo, the veteran, is going on 27 years in the business. Bos past apprentice and Jimbos son Billy also works at UAF and has training and coaching from both Bo and Jimbo. He doesnt need it but this is a family here and we make sure we emphasize that during business hours. Dont forget Rob, the Body Piercer, whom was also apprenticed under Bo. This is a business that is relaxed and laid back. We make sure our customers are taken care of before, during and after each procedure. All the nay-sayer speak of people whom have come and past through these doors. Dont take my word for it, Come see us and see for yourself. We are open EVERYDAY of the week.

Decent selection in body jewelry which is why I went there. Not for tattoos. They had titanium jewelry as well. Which they were able to change the color of the metal. If your having an allergic reaction to jewelry titanium is the way to go. Thanks guysI will be back
